| Moodle FERPA Acknowledgment Form (pdf 99k) | To get access to Moodle, undergraduate TAs and ‘unofficial’ graduate and undergraduate TAs must be listed in SPIRE as Student Assistants (SPK/MDL Only) (shorthand for SPARK/Moodle Only) first. All students who need this listing must read and sign this form. Sponsoring faculty members must provide to the Scheduling Representatives in their department a separate copy of this form for every student who requires access.
