To get access to SPARK, undergraduate TAs and ‘unofficial’ graduate and undergraduate TAs must be listed in SPIRE as Student Assistants (SPK/MDL Only) (shorthand for SPARK/Moodle Only) first. All students who need this listing must read and sign this form. Sponsoring faculty members must provide a separate copy of this form for every student who requires access. | CGI Server Contract (pdf, 110k) |
Web site administrators can use OIT's CGI server to add CGI (Common Gateway Interface) forms to their Web sites. Access to the CGI server is only available to Information Providers who maintain department, organization, or course-related Web sites. In addition to this form, applicants must also have an Information Provider Contract on file with OIT. |
