Members of the University community and their guests may need to apply to access certain restricted services. The forms below are often part of a multi-step application process. Check the appropriate section of the Web site for the most recent submission instructions.

Non-Employee, Non-Student (NENS) Application (pdf, 107K) Courtesy (NENS) accounts are provided to certain individuals who have a special relationship with the University, usually for a limited time. Courtesy Accounts for Non-Employees, Non-Students (NENS)
SPIRE Information & Authorization Form (pdf, 114k)                     

Staff, instructors, and select students are required to fill out this form in order to receive administrative access to SPIRE. Administrative Access to SPIRE

Moodle FERPA Acknowledgment Form (pdf 99k) To get access to Moodle, undergraduate TAs and ‘unofficial’ graduate and undergraduate TAs must be listed in SPIRE as Student Assistants (SPK/MDL Only) (shorthand for SPARK/Moodle Only) first. All students who need this listing must read and sign this form. Sponsoring faculty members must provide to the Scheduling Representatives in their department a separate copy of this form for every student who requires access. Information Provider Contract (pdf, 54k) Each department or organization requesting an official University Web site must have a signed Information Provider Contract on file with the Office of Information Technologies (OIT).
